Sometimes a VBA screen customization that creates a new custom control works correctly in development but crashes when installing from a there to a test or production environment. Everything in the customization may work when the control (and references to it) are removed. But if the control is a requirement, that’s not much help.

If everything else seems to be correct it’s sometimes helpful to export all the other customizations on that form with VBA as text. Save, then import your new customization, then re-import the others you just saved. I’ve seen that work several times.
